Our 12-month evidence-based programme focuses on lifestyle modifications through personalised coaching, complementing the clinical recommendations of your General Practitioner.  This approach aims to help you effectively manage these challenging conditions, and ensures you are never alone with continuous support.

How to apply?

This programme is exclusively available at no additional cost to AIA customers who are 40 years old and above and have diabetes, high cholesterol or high blood pressure. Simply be one of the first 1,500 to activate this programme on Amplify health's Amped app.

  • You will receive an email requesting your health screening results from within the last 3 months, which must meet our eligibility criteria.

If you're eligible, Amplify Health will email you your new Amped app account details. Download and register on Amped

Start your consultation


Complete your first video or text-based consultation with your coach and have at least 1 personalised health goal set.

Here are some additional services that you might be interested in:
Basic health screening
If you do not have a recent health screening report, WhiteCoat provides a complimentary basic health screening for the first 500 customers. Beyond the first 500 customers, the basic health screening will be available at a negotiated rate of S$45 (before GST). 
 
Simply indicate your interest in a health screening with WhiteCoat when registering for the programme with Amplify Health. 
 
 
FreeStyle Libre – Continuous Glucose Monitoring Sensor
Aside to this programme, you could get a Freestyle Libre Sensor at S$10 (U.P S$103.10) from Abbott to help you track glucose levels in real time with zero finger pricks.

The Chronic Disease Management Programme is open to the first 1,500 'activated' customers. The 'activated' customer is defined as an AIA insured customer who has completed the following: 
 
  • Submitted his/her health screening result to Amplify Health and has received confirmation of his/her eligibility for the Chronic Disease Management Programme 
  • Completed a first video or text-based consultation with the coach and has at least 1 personalised health goal set by the coach

The Chronic Disease Management Programme is not a contractual benefit. Amplify Health Asia Pte. Limited, WhiteCoat Holdings Pte. Ltd. and Abbott Laboratories (Singapore) Private Limited are independent third-party companies ("Healthcare Partners"). AIA Singapore Private Limited ("AIA") shall not be responsible or liable in any way for any act, omission, advice or information that is given, or statements made by Healthcare Partners or other persons (including healthcare providers) arising from or in connection with the provision of the medical services, products, any other value-added services provided by Healthcare Partners or any marketing activities conducted by Healthcare Partners. AIA reserves the right, at its sole and absolute discretion, to cancel, terminate and/or withdraw this value-added service any time or for any reason without notice.  
 
Any and all personal data provided to Amplify Health Asia Pte. Limited, WhiteCoat Holdings Pte. Ltd. and AIA will be used to ascertain your eligibility to access the services and other information herein.
